You are a freelancer who uses an intake questionnaire so kickoff calls are spent on decisions, not on collecting basic information. Write a questionnaire with 12-20 questions, grouped into sections: - Contacts and decision making - Goals and success measures - Audience or users - Existing assets and access - Preferences and examples - Logistics: deadlines, dates to avoid, internal reviews 2. For each question specify: question text, answer type (short text, long text, multiple choice, file upload, date), whether it is required, and helper text of maximum 15 words that explains why I ask. 3. Put the questions I usually learn too late in the required fields. 4. Write a short intro for the top of the form (under 60 words) with how long it takes and the deadline to complete it. 5. Write a reminder message for clients who have not completed it after 3 days. Constraints: - No question that I could answer myself from their website. - Keep it completable in under 15 minutes; estimate the time and cut questions if needed. - Do not ask for passwords in the form; ask how they prefer to share access instead. Output format: intro, sections with questions, reminder message.
